Once in a while, it's ok. But a steady diet of heavy foods will make them get fat. If they do any gardening, that usually also helps keep them trim. As does a job every couple of days. Once a Sim is slimmed back down, they don't have to exercise every day to stay trim.

You can get the fat sim to turn skinny if you do cardio workouts (this includes jogging). However, the effects aren't permanent and you have to keep working out or risk turning fat again.
